TO RESERVE YOUR CHILD’S ENROLLMENT FOR 2022 - 2023, PLEASE  FILL OUT THE INTENT TO ENROLL FORM.

Note: All documents [Birth Certificate, Immunization Record, Transcript, & if applicable, Individualized Education Plan (IEP), 504 Plan and/or any Student Support Information] can be submitted via the school site office.

To enroll:

Step 1: Call, Text, or Email the Community Outreach Manager at 

(408)729-2281 ext. 1068/[email protected]

Step 2: Schedule an appointment with the LVLA Staff and Community Outreach Manager.

Step 3: Arrive prepared with your child’s Birth Certificate, Immunization Record, and Current Transcript. **

If applicable, please include Individualized Education Plan (IEP), 504 Plan and/or any Student Support Information.
